What is a Family Physician?
A family physician is a doctor who takes care of your whole family. Family physicians create caring relationships with patients and their families. They really get know their patients. They listen to them and help them make the right health care decisions.

What are your office hours?
Monday through Friday 7a.m.-5 p.m.

What ages do you treat?
We treat newborn to geriatric patients.

What if I need to see the doctor right away?
We try to accommodate same day appointments in emergent situations. Please contact the office at 480-988-1659 and speak with the receptionist or a nurse.


If it is a true life-threatening emergency, please go to the nearest Hospital Emergency Room for treatment or dial 9-1-1.

What is a family nurse practitioner?
A family nurse practitioner is a nurse who has a masters degree in nursing and has completed intership in all specialties, focusing on family medicine. They are licensed by the state of Arizona and are allowed to treat patients and prescribe medications.

What information do I need to bring to my appointment?
If you are a new patient, be sure to fill out the forms found in the form section of our website, this will save you time in the office. You should also bring your insurance card, driver’s license or a picture ID.

What do I do if I need to change or cancel an appointment?
Please call the office in advance, when possible. If you do not contact us regarding a cancellation, we do charge a cancellation fee.

What if I don’t have insurance?
We will work with you to make financial arrangements if necessary. Please be aware that you will be required to pay for your initial office visit at that time. Please contact our office staff regarding questions about payment arrangements.

How do I get a refill prescription?
Currently, you will call the office and leave a message for the nurse, but soon you will be able to access our on-line refill prescription request center.

What about information on the internet? Is that helpful to discuss with my doctor?
Any health concerns that you have should be discussed with Dr. Smith or the Family Nurse Practitioner, they will be able to let you know if the information you’ve found is appropriate for your own medical health.
